Panicking about ability to get a doctors certificate...?

Your doctor should be willing to write you a note based on the symptoms you've described. Not only have you been extremely ill, you are now forced to grapple with a rather serious medical diagnosis. You will have to ask for the note though, but I highly recommend that you ask because as you said it would be a terrible waste if you did poorly because you have not been in any shape to study.
